Contest is open to all residents of the USA between the ages of 5 and 12 years old. Void where prohibited by law. Winner will be chosen solely by LKTV Productions. Employees of LKTV Productions, its sponsors, exhibitors, and other television, radio or print media, and their affiliates, subsidiaries and advertising agencies and their immediate family members are not eligible to claim prize. By entering this contest, participants understand and agree to grant full permission to LKTV Productions for unlimited use of their video, a portion of their video and/or their likeness to be used by LKTV Productions to promote Lucky Kat TV and LKTV on the Internet and in various printed materials. All participants further understand and agree that no compensation has been promised or is owed to participants. LKTV Productions adheres to the strictest of Privacy Policies and is fully COPPA compliant. No video that shares personal or private information, that violates LKTV Production's Privacy Policy or would risk a child's safety in any way will be accepted as a qualifying entry. Only qualifying entries will be used for promotional purposes. A prizewinner list may be obtained by sending a self-addressed, stamped envelope to LKTV Productions, 121 W. Lexington Drive, Suite 232, Glendale, CA 91203. All requests for winner lists must be mailed and received by LKTV after the contest is over but prior to 90 days after the contest has been concluded. Odds of winning depend on the number of qualifying entries received. Any prize that is awarded in the form of a gift certificate may have an expiration date and use of such certificate may be based on availability. Gift certificates are only redeemable at the locations for which they are specified. Winners are responsible for paying all applicable local, county, state and federal taxes on prizes based on the estimated retail value of the prize, as set forth in the contest rules, and will be issued an IRS 1099 form for all prizes won from LKTV where the aggregate value of all prizes is $600 or more. Winner will be notified by phone, e-mail and/or certified mail. Prize must be claimed within 15 days of notification. In the event of noncompliance, an alternative winner may be selected. Prize winner will assume responsibility for all taxes, if applicable. Sponsor(s) of this package are not liable for injury, loss or damage of any kind resulting from the acceptance or use of prize and participation in this promotion. Entrants may be contacted by phone, mail or e-mail for purposes of promoting various products and/or services. Winner must sign approved waiver forms prior to claiming prize.
